  • The Dashboard summarizes reported health information entered by parents of children with medical complexity (CMC) using the MyChildCMC App for home monitoring. It is intended to be used by healthcare professionals as a source of information to augment patient care management. The information, content and data presented in this dashboard (e.g., as text, graphics, and images) are for informational and clinical-decision support purposes only and should not be used for diagnosing purposes or be a substitute for usual medical care.
  • The MyChildCMC app is a service provided to parents of CMC, at no cost, for daily monitoring of common symptoms experienced by CMC. MyChildCMC app is intended for home monitoring use only. MyChildCMC app is not intended to provide a diagnosis, or to suggest appropriate treatment, for any medical disorder. Patients and/or their parents assume all risks associated with their use of the MyChildCMC app.
  • Parents using MyChildCMC app are solely responsible for checking their child’s vital signs and symptoms, completing the on-line questionnaire with the recommended frequency, and providing complete and accurate information. It is your responsibility to exercise professional medical judgment, and to research the accuracy, completeness and usefulness of information provided through other sources or direct patient/parent consultation.
  • The Dashboard should not be used for emergency medical treatment or to transmit urgent information. In case of a medical emergency, please follow your established emergency protocols.
